STEP ONE:

Download the Application for Waitlist form and turn it in to the school.

-For full-time students, fax to 801-465-7808 or bring the form down to the school’s registrar office.

-For CAP students, contact Mrs. Denning at cap@libertyacademycs.org to set up a One-On-One parent meeting or fax the form to her attention at 801-465-7808
